T\u1eeb vi\u1ec7c <strong>t\u1ea7m so\u00e1t ung th\u01b0<\/strong>, <strong>x\u00e9t nghi\u1ec7m tr\u01b0\u1edbc sinh kh\u00f4ng x\u00e2m l\u1ea5n (NIPT)<\/strong> cho \u0111\u1ebfn <strong>theo d\u00f5i sau c\u1ea5y gh\u00e9p<\/strong>, cfDNA ng\u00e0y c\u00e0ng ch\u1ee9ng t\u1ecf vai tr\u00f2 quan tr\u1ecdng trong c\u00e1c x\u00e9t nghi\u1ec7m ch\u00ednh x\u00e1c v\u00e0 an to\u00e0n h\u01a1n cho ng\u01b0\u1eddi b\u1ec7nh. Trong b\u00e0i vi\u1ebft n\u00e0y, ch\u00fang ta s\u1ebd c\u00f9ng t\u00ecm hi\u1ec3u v\u1ec1 <strong>xu h\u01b0\u1edbng \u1ee9ng d\u1ee5ng cfDNA trong NGS<\/strong>, nh\u1eefng \u01b0u \u0111i\u1ec3m v\u01b0\u1ee3t tr\u1ed9i, c\u00e1c th\u00e1ch th\u1ee9c k\u1ef9 thu\u1eadt, v\u00e0 ti\u1ec1m n\u0103ng ph\u00e1t tri\u1ec3n trong t\u01b0\u01a1ng lai \u2013 \u0111\u1eb7c bi\u1ec7t trong b\u1ed1i c\u1ea3nh Vi\u1ec7t Nam \u0111ang t\u1eebng b\u01b0\u1edbc c\u1eadp nh\u1eadt c\u00e1c c\u00f4ng ngh\u1ec7 sinh h\u1ecdc ti\u00ean ti\u1ebfn n\u00e0y.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" style=\"font-size:25px\"><strong>1. cfDNA l\u00e0 g\u00ec? Ngu\u1ed3n g\u1ed1c v\u00e0 \u00fd ngh\u0129a l\u00e2m s\u00e0ng<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><strong><a href=\"https:\/\/abtvn.com\/en\/cfdna-cell-free-dna-trong-chan-doan-ung-thu-va-ung-dung\/\" data-type=\"link\" data-id=\"https:\/\/abtvn.com\/cfdna-cell-free-dna-trong-chan-doan-ung-thu-va-ung-dung\/\">cfDNA (cell-free DNA)<\/a><\/strong> l\u00e0 c\u00e1c \u0111o\u1ea1n DNA ng\u1eafn (th\u01b0\u1eddng t\u1eeb 150-200 base pairs) t\u1ed3n t\u1ea1i t\u1ef1 do trong huy\u1ebft t\u01b0\u01a1ng ho\u1eb7c c\u00e1c d\u1ecbch c\u01a1 th\u1ec3 m\u00e0 kh\u00f4ng n\u1eb1m trong t\u1ebf b\u00e0o. Ch\u00fang \u0111\u01b0\u1ee3c gi\u1ea3i ph\u00f3ng t\u1eeb c\u00e1c t\u1ebf b\u00e0o \u0111ang ch\u1ebft theo c\u01a1 ch\u1ebf t\u1ef1 nhi\u00ean (apoptosis) ho\u1eb7c ho\u1ea1i t\u1eed. \u1ede ng\u01b0\u1eddi kh\u1ecfe m\u1ea1nh, cfDNA ch\u1ee7 y\u1ebfu \u0111\u1ebfn t\u1eeb c\u00e1c t\u1ebf b\u00e0o m\u00e1u. Next-Generation Sequencing (NGS) v\u00e0 kh\u1ea3 n\u0103ng \u1ee9ng d\u1ee5ng trong ph\u00e2n t\u00edch cfDNA<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Next-Generation Sequencing (NGS) <\/strong>l\u00e0 c\u00f4ng ngh\u1ec7 gi\u1ea3i tr\u00ecnh t\u1ef1 gen th\u1ebf h\u1ec7 m\u1edbi cho ph\u00e9p ph\u00e2n t\u00edch h\u00e0ng tri\u1ec7u \u0111o\u1ea1n DNA \u0111\u1ed3ng th\u1eddi v\u1edbi \u0111\u1ed9 ch\u00ednh x\u00e1c cao v\u00e0 chi ph\u00ed ng\u00e0y c\u00e0ng gi\u1ea3m. Khi k\u1ebft h\u1ee3p v\u1edbi cfDNA, NGS mang l\u1ea1i nhi\u1ec1u l\u1ee3i th\u1ebf v\u01b0\u1ee3t tr\u1ed9i:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Ph\u00e1t hi\u1ec7n c\u00e1c bi\u1ebfn th\u1ec3 di truy\u1ec1n hi\u1ebfm v\u1edbi \u0111\u1ed9 nh\u1ea1y cao.<\/li>\n\n\n\n<li>Cho ph\u00e9p ph\u00e2n t\u00edch kh\u00f4ng x\u00e2m l\u1ea5n t\u1eeb m\u1eabu m\u00e1u ngo\u1ea1i vi.<\/li>\n\n\n\n<li>\u00c1p d\u1ee5ng r\u1ed9ng r\u00e3i trong nhi\u1ec1u l\u0129nh v\u1ef1c: s\u1ea3n khoa, ung th\u01b0 h\u1ecdc, gh\u00e9p t\u1ea1ng, nhi\u1ec5m tr\u00f9ng&#8230;<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\" style=\"font-size:25px\"><strong>3. C\u00e1c xu h\u01b0\u1edbng \u1ee9ng d\u1ee5ng cfDNA k\u1ebft h\u1ee3p NGS n\u1ed5i b\u1eadt<\/strong><\/h2>\n\n\n\n<h3 class=\"wp-block-heading\" style=\"font-size:22px\"><strong>3.1. X\u00e9t nghi\u1ec7m tr\u01b0\u1edbc sinh kh\u00f4ng x\u00e2m l\u1ea5n (NIPT)<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>NIPT (Non-Invasive Prenatal Testing)<\/strong> l\u00e0 \u1ee9ng d\u1ee5ng s\u1edbm v\u00e0 ph\u1ed5 bi\u1ebfn nh\u1ea5t c\u1ee7a cfDNA trong l\u00e2m s\u00e0ng. T\u1eeb tu\u1ea7n thai th\u1ee9 10, cfDNA thai nhi (cffDNA) \u0111\u00e3 hi\u1ec7n di\u1ec7n trong m\u00e1u m\u1eb9 v\u00e0 c\u00f3 th\u1ec3 chi\u1ebfm 5\u201320% t\u1ed5ng l\u01b0\u1ee3ng cfDNA. V\u1edbi s\u1ef1 h\u1ed7 tr\u1ee3 c\u1ee7a NGS, x\u00e9t nghi\u1ec7m NIPT c\u00f3 th\u1ec3:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>S\u00e0ng l\u1ecdc h\u1ed9i ch\u1ee9ng Down (trisomy 21), Edwards (trisomy 18), v\u00e0 Patau (trisomy 13) v\u1edbi \u0111\u1ed9 ch\u00ednh x\u00e1c tr\u00ean 99%.<\/li>\n\n\n\n<li>M\u1edf r\u1ed9ng sang c\u00e1c r\u1ed1i lo\u1ea1n \u0111\u01a1n gen (monogenic disorders) nh\u01b0 x\u01a1 nang, thi\u1ebfu m\u00e1u h\u1ed3ng c\u1ea7u h\u00ecnh li\u1ec1m.<\/li>\n\n\n\n<li>K\u1ebft h\u1ee3p AI \u0111\u1ec3 ph\u00e2n t\u00edch d\u1eef li\u1ec7u, gi\u1ea3m d\u01b0\u01a1ng t\u00ednh gi\u1ea3.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Xu h\u01b0\u1edbng hi\u1ec7n nay l\u00e0 gi\u1ea3m chi ph\u00ed x\u00e9t nghi\u1ec7m v\u00e0 t\u00edch h\u1ee3p NIPT v\u00e0o h\u1ec7 th\u1ed1ng y t\u1ebf c\u1ed9ng \u0111\u1ed3ng, \u0111\u1eb7c bi\u1ec7t \u1edf c\u00e1c qu\u1ed1c gia \u0111ang ph\u00e1t tri\u1ec3n nh\u01b0 Vi\u1ec7t Nam.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" style=\"font-size:22px\"><strong>3.2. T\u1ea7m so\u00e1t v\u00e0 qu\u1ea3n l\u00fd ung th\u01b0<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>cfDNA, <\/strong>\u0111\u1eb7c bi\u1ec7t l\u00e0 <strong>ctDNA<\/strong>, \u0111ang c\u00e1ch m\u1ea1ng h\u00f3a l\u0129nh v\u1ef1c ung th\u01b0 h\u1ecdc th\u00f4ng qua kh\u00e1i ni\u1ec7m &#8220;sinh thi\u1ebft l\u1ecfng&#8221; (liquid biopsy). ctDNA mang c\u00e1c bi\u1ebfn th\u1ec3 di truy\u1ec1n \u0111\u1eb7c tr\u01b0ng c\u1ee7a kh\u1ed1i u (\u0111\u1ed9t bi\u1ebfn, methyl h\u00f3a DNA, ho\u1eb7c thay \u0111\u1ed5i c\u1ea5u tr\u00fac) v\u00e0 c\u00f3 th\u1ec3 \u0111\u01b0\u1ee3c ph\u00e1t hi\u1ec7n trong m\u00e1u ngay c\u1ea3 khi kh\u1ed1i u c\u00f2n r\u1ea5t nh\u1ecf.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>T\u1ea7m so\u00e1t s\u1edbm:<\/strong> C\u00f4ng ngh\u1ec7 SPOT-MAS (Screening for the Presence Of Tumor by Methylation And Size) \u0111ang \u0111\u01b0\u1ee3c \u1ee9ng d\u1ee5ng \u0111\u1ec3 ph\u00e1t hi\u1ec7n d\u1ea5u hi\u1ec7u ung th\u01b0 tr\u01b0\u1edbc khi c\u00f3 tri\u1ec7u ch\u1ee9ng.<\/li>\n\n\n\n<li><strong>Theo d\u00f5i \u0111i\u1ec1u tr\u1ecb:<\/strong> Ph\u00e2n t\u00edch ctDNA gi\u00fap \u0111\u00e1nh gi\u00e1 \u0111\u00e1p \u1ee9ng \u0111i\u1ec1u tr\u1ecb, ph\u00e1t hi\u1ec7n kh\u00e1ng thu\u1ed1c s\u1edbm (nh\u01b0 \u0111\u1ed9t bi\u1ebfn EGFR T790M).<\/li>\n\n\n\n<li><strong>Y h\u1ecdc c\u00e1 th\u1ec3 h\u00f3a:<\/strong> D\u1ef1a v\u00e0o \u0111\u1eb7c \u0111i\u1ec3m gen, b\u00e1c s\u0129 c\u00f3 th\u1ec3 x\u00e2y d\u1ef1ng ph\u00e1c \u0111\u1ed3 \u0111i\u1ec1u tr\u1ecb ri\u00eang bi\u1ec7t.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\" style=\"font-size:22px\"><strong>3.3. Theo d\u00f5i sau gh\u00e9p t\u1ea1ng b\u1eb1ng cfDNA<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Donor-derived cfDNA (dd-cfDNA) <\/strong>l\u00e0 c\u00e1c \u0111o\u1ea1n DNA t\u1eeb c\u01a1 quan gh\u00e9p. Khi x\u1ea3y ra hi\u1ec7n t\u01b0\u1ee3ng th\u1ea3i gh\u00e9p, n\u1ed3ng \u0111\u1ed9 dd-cfDNA trong m\u00e1u ng\u01b0\u1eddi nh\u1eadn t\u0103ng l\u00ean r\u00f5 r\u1ec7t. NGS gi\u00fap \u0111\u1ecbnh l\u01b0\u1ee3ng dd-cfDNA ch\u00ednh x\u00e1c \u0111\u1ec3:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Ph\u00e1t hi\u1ec7n s\u1edbm th\u1ea3i gh\u00e9p tr\u01b0\u1edbc khi c\u00f3 bi\u1ec3u hi\u1ec7n l\u00e2m s\u00e0ng.<\/li>\n\n\n\n<li>Gi\u1ea3m nhu c\u1ea7u sinh thi\u1ebft x\u00e2m l\u1ea5n.<\/li>\n\n\n\n<li>T\u0103ng c\u01b0\u1eddng theo d\u00f5i th\u01b0\u1eddng quy, c\u1ea3i thi\u1ec7n k\u1ebft qu\u1ea3 c\u1ea5y gh\u00e9p.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">M\u1ed9t s\u1ed1 x\u00e9t nghi\u1ec7m th\u01b0\u01a1ng m\u1ea1i nh\u01b0 AlloSure, AlloSeq \u0111\u00e3 \u0111\u01b0\u1ee3c ph\u00e1t tri\u1ec3n v\u00e0 ch\u1ee9ng minh hi\u1ec7u qu\u1ea3 trong theo d\u00f5i th\u1ea3i gh\u00e9p tim, gan v\u00e0 th\u1eadn.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" style=\"font-size:22px\"><strong>3.4. cfDNA trong c\u00e1c b\u1ec7nh l\u00fd tim m\u1ea1ch, nhi\u1ec5m tr\u00f9ng v\u00e0 t\u1ef1 mi\u1ec5n<\/strong><\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>B\u1ec7nh tim m\u1ea1ch:<\/strong> Ph\u00e1t hi\u1ec7n DNA t\u1eeb t\u1ebf b\u00e0o c\u01a1 tim b\u1ecb t\u1ed5n th\u01b0\u01a1ng trong nh\u1ed3i m\u00e1u c\u01a1 tim.<\/li>\n\n\n\n<li><strong>Nhi\u1ec5m tr\u00f9ng:<\/strong> X\u00e1c \u0111\u1ecbnh t\u00e1c nh\u00e2n g\u00e2y b\u1ec7nh b\u1eb1ng cfDNA vi sinh v\u1eadt qua NGS.<\/li>\n\n\n\n<li><strong>B\u1ec7nh t\u1ef1 mi\u1ec5n:<\/strong> Ph\u00e2n t\u00edch cfDNA gi\u00fap hi\u1ec3u r\u00f5 h\u01a1n c\u01a1 ch\u1ebf b\u1ec7nh l\u00fd nh\u01b0 lupus ban \u0111\u1ecf h\u1ec7 th\u1ed1ng.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\" style=\"font-size:25px\"><strong>4. \u01afu \u0111i\u1ec3m v\u00e0 th\u00e1ch th\u1ee9c khi \u1ee9ng d\u1ee5ng cfDNA v\u1edbi NGS<\/strong><\/h3>\n\n\n\n<h4 class=\"wp-block-heading\" style=\"font-size:20px\"><strong>4.1. \u01afu \u0111i\u1ec3m<\/strong><\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Kh\u00f4ng x\u00e2m l\u1ea5n:<\/strong> M\u1eabu l\u1ea5y t\u1eeb m\u00e1u ngo\u1ea1i vi, an to\u00e0n v\u00e0 d\u1ec5 th\u1ef1c hi\u1ec7n.<\/li>\n\n\n\n<li><strong>\u0110\u1ed9 nh\u1ea1y cao:<\/strong> Ph\u00e1t hi\u1ec7n \u0111\u01b0\u1ee3c bi\u1ebfn \u0111\u1ed5i gen v\u1edbi t\u1ea7n su\u1ea5t r\u1ea5t th\u1ea5p.<\/li>\n\n\n\n<li><strong>\u1ee8ng d\u1ee5ng r\u1ed9ng:<\/strong> T\u1eeb ch\u1ea9n \u0111o\u00e1n s\u1edbm \u0111\u1ebfn theo d\u00f5i \u0111i\u1ec1u tr\u1ecb v\u00e0 ph\u1ee5c h\u1ed3i.<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\" style=\"font-size:20px\"><strong>4.2. Th\u00e1ch th\u1ee9c<\/strong><\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Chi ph\u00ed cao:<\/strong> Thi\u1ebft b\u1ecb NGS v\u00e0 ph\u00e2n t\u00edch d\u1eef li\u1ec7u y\u00eau c\u1ea7u \u0111\u1ea7u t\u01b0 l\u1edbn.<\/li>\n\n\n\n<li><strong>Y\u00eau c\u1ea7u k\u1ef9 thu\u1eadt cao:<\/strong> cfDNA d\u1ec5 ph\u00e2n m\u1ea3nh, c\u1ea7n quy tr\u00ecnh x\u1eed l\u00fd chu\u1ea9n h\u00f3a.<\/li>\n\n\n\n<li><strong>Ph\u00e2n t\u00edch d\u1eef li\u1ec7u ph\u1ee9c t\u1ea1p:<\/strong> C\u1ea7n \u0111\u1ed9i ng\u0169 chuy\u00ean gia sinh tin h\u1ecdc v\u00e0 h\u1ea1 t\u1ea7ng t\u00ednh to\u00e1n m\u1ea1nh.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">cfDNA k\u1ebft h\u1ee3p c\u00f4ng ngh\u1ec7 NGS \u0111ang m\u1edf ra k\u1ef7 nguy\u00ean ch\u1ea9n \u0111o\u00e1n kh\u00f4ng x\u00e2m l\u1ea5n ch\u00ednh x\u00e1c v\u00e0 to\u00e0n di\u1ec7n.
